Who could have foreseen it?

A tourist boat in India left at least twenty-two of its around fifty passengers dead when it capsized at night. The double-decker boat was twofold over its capacity.

Many of the passengers were not wearing life jackets at the time of the incident, survivors told local media.

That would, at least in part, be because there were twice as many passengers as could be safely carried. Even if life jackets were provided—never to be assumed—there would'nt've been enough to go 'round anyway. Innit? (rolleyes)

Authorities are investigating the cause of the mishap.

No, this is not a mishap. A mishap is a minor accident of little consequence. This is a tragedy. A tragedy stemming from clear disregard for safety, and probably also criminal breach of operational permits; and which, as a result, has claimed many innocent lives.
